What happened?: No opt-in sample was found on your website, this is the main rejection reason. 
How can this be fixed? In that sense, to ensure compliance and approval, a proper opt-in should look like this and must be located in the same section. where you are collecting your end-users phone number informationIn other words, it should include
-Consent: Clearly stating that providing a phone number means agreeing to receive messages.
